The abortion pill procedure is used from 3 to 24 weeks or even more - Tablets for abortion (RU486, Mifeprex, and Mifepristone) and Misoprostol (Cytotec) are the two medications generally used for performing the abortion pill or medical abortion procedure from 1 week; 2 weeks; 1 month; 2 months and up to 10 to 14 weeks (3 months) gestation. These medical abortion methods are used in the second (14, 16 [4 months], 18, 20, 22, 24 [6 months] 26 to 28 weeks) and third trimester of pregnancy (further than 28 weeks [7 months], 30, 32, 34 weeks) if the pregnancy poses a threat to the mother’s life or health or if there is a significant fetal abnormality or genetic defect, and for incidences of rape or incest.
Prior to the approval of RU486, the combination of Methotrexate and Misoprostol was used in the first trimester for non-surgical chemical abortion procedures. RU486 is not available in a Pharmacy. It is only offered at hospitals, physician offices or abortion clinics. In the United States it is illegal to order RU486 or other abortion pills online. The FDA has issued a statement advising women not to order home abortion kits because of the risk of not knowing what the medications are or whether they are the appropriate dosages. They also emphasized that all abortions should be done only under medical supervision because of the rare but real risks that can occur including maternal death.
The number of weeks, months, how late, how far, how long or the cutoff date allowing for an abortion or termination of pregnancy to be legally performed is vague and imprecise. Abortions after the 20th or 21st weeks of pregnancy are now illegal in over 36 States. Women who are well off and have the ability to travel to other States or out of the country are not burdened by any of the outlandish laws created specifically for the purpose of hindering or preventing abortions.

The medications used are RU486 (Mifeprex, Mifepristone, Mifegyne) and Misoprostol (Cytotec). RU486 is an anti-progestin which binds to the progesterone receptors of the uterus causing sloughing and necrosis of the blood vessels leading to increased pressure and uterine contractions. In early pregnancies, this is responsible for separating the pregnancy from the uterine wall which can lead to 40 to 60% of abortion alone. Misoprostol (Cytotec) is a prostaglandin that prevents ulcers from occurring in the stomach when patients take non-steroidal anti-inflammatories. Misoprostol works by combining the prostaglandin receptors of the uterine wall and causing contractions.
There is a 90 to near 100% success rate using the combination of RU486 and Misoprostol tablets for terminating early pregnancies. (3 to 14 weeks) The success rate depends on many factors. Just as there are different ways to perform surgical procedures, there are different methods to perform the medical abortion or abortion pill procedure.
Taking Misoprostol tablets alone can terminate pregnancies from 3 to 34 weeks nearly 100% of the time if the proper instructions and steps are followed. Complications are rarely seen at the Women’s Centers Same Day Abortion Clinic Services for patients who are 3 to 24 weeks. Patients between 3 to 12 weeks gestation complete their procedure within 4 to 6 hours. Patients up to 24 weeks generally complete their procedures in less than 12 hours, although it can be as much as 24 hours before their procedure is complete. Patients from 3 to 24 weeks have completed the procedure in 30 minutes; however, this is unusual. There is no surgery, no sedation and no need for patients to bring someone with them. There is no need for a one or two week follow-up sonogram because with the One Day Abortion procedure, a sonogram is performed prior to discharge from the office. Patients who do not wish to stay or can’t stay after taking the medication in the office are allowed to return the following day or another day agreed upon if they are generally less than 12 weeks gestation. Pain medications are rarely if ever required.
The medication is simple to take and the instructions are easy to follow but must be done precisely in order to be 100% effective. The reasons most patients choose to initiate the abortion pill or medical abortion process at our clinics: 1) they do not want to risk an incomplete procedure; 2) they do not have anyone at home to assist or help them; 3) they want the utmost in privacy and do not want anyone to know they are going through this procedure; 4) they like the idea of the abortion pill procedure but prefer a one day process over the regular abortion pill procedure which consists of taking two sets of medications over a 48 hour period and requires returning to the office for a follow-up sonogram in one or two weeks.

Mid to late second trimester abortion procedures and late term abortion procedures with RU486 and Misoprostol or with Misoprostol alone generally occur by using the following process:
Patients 15 weeks or further are given medications to start on their first visit. Those who are 21.5 weeks or further are given medications to stop the fetal heartbeat prior to being discharged. Patients return the following morning to complete the induction procedure. After monitoring to assure no complications (<1%), the patient is discharged.
The question of performing a pill abortion using RU486 vs Surgery is commonly asked. Women fear having complications from surgery. No matter how many weeks they are, if there is a chance of uterine perforation, one would choose the safer method no matter how early or far the woman is in her pregnancy. Although surgical abortion is safe, there are complications that can occur which include perforation of the uterus, cervical tears, bowel and bladder injury, damage to the fallopian tubes and ovaries. Complications are rare with medical procedures. Neither surgical or RU486 procedures affect future pregnancies unless complications occur such as Pelvic Inflammatory Disease which is extremely rare with today’s safe practices.
Physicians will not be able to determine an abortion was performed. This privacy and confidentiality is absolutely necessary. Women early in pregnancy generally know when they are having a miscarriage and feel no need to call a doctor. Should a woman not wish to inform anyone she had an abortion procedure, but may feel it necessary to explain why she is no longer pregnant, she may say she is having or has had a miscarriage.
Side effects and complications commonly associated with the Same Day Abortion Procedure may include the following: Headaches, fever and chills, nausea and vomiting, lower back and abdominal discomfort and vaginal bleeding, incomplete abortions, failed abortions and pelvic infection. These conditions can be resolved or diminished with medications (usually misoprostol and antibiotics). Should a surgical procedure become necessary (less than 1% of the time), Advanced IV Sedation can be administered. Rare risks and complications associated with RU486 and Misoprostol are excessive vaginal bleeding requiring a blood transfusion (0.1 to 0.2); severe sepsis requiring immediate hospitalization and extremely rare is maternal death. The latter is caused by bacteria which enters the blood stream. This bacteria (C Sordellii) has also caused death in women who have had normal vaginal deliveries at term, spontaneous miscarriages and C-Sections.
