1. No aspirin products or use of street drugs for 48 hours prior to appointment.
2. Bring valid government issued ID, such as a driver’s license.
3. Take a shower or bath with your favorite music playing to help relax and decrease anxiety.
4. Wear regular underwear (no thongs).
5. Do not eat or drink anything for four (4) hours prior to your appointment time. If earlier than 6 weeks and not having IV sedation, you may eat and drink up to the time of your appointment.
6. Expect to be at the Offices for 3 to 6 hours.
7. In order to receive IV Sedation, Deep Sedation, or General Anesthesia, you must have someone drive you home. We ask that you not drive or operate machinery for 12 to 24 hours
8. Please bring only one person if at all possible. They must have a valid government issued photo ID to remain in the office.
9. Children are not permitted in the office.
10. No purses, backpacks or bags of any kind are allowed in our offices except for patients who will stay overnight.
11. Payment is expected in full at the time of service. We accept cash, traveler’s checks, money orders, wired funds, Visa, MasterCard, American Express, or Discover card.
12. You also have the option to pay online.
